About the Role:
As a Housing Case Manager at SARC, you will play a vital role in aiding individuals and families requiring assistance. Your responsibilities will include:
- Conduct case management duties including: completing intakes, service plans, case notes, needs and strengths assessments, screenings, referrals and advocacy.
- Initial Case Management Profile and Individual Services plan for all general case management clients.
- Aid in locating and securing housing, connection to community resources, government and state benefits, medical, mental health and substance use services.
- Remove barriers to permanent housing; e.g., acquisition of identification, legal aid, benefits/income assistance, transportation to appointments
- Complete Housing Stability and Rental Assistance Plan for all clients to assist with planning for long term housing.
- Identify permanent housing units to move individuals into housing as quickly as possible once linked to a rental subsidy
- Client determined to be appropriate for SARC rental assistance should have a completed Welcome Home Client Contract, What Does My Lease Say, and Monthly Budget Form.
- Locate, identify and utilize partner agencies, social services and resources for families.
- Meet regularly with identified clients to provide advocacy, information, referrals, resources, safety planning, and supportive counseling.
- Advocate with other social service agencies, shelters, mental health facilities, housing providers, schools, and similar program personnel on behalf clients.
- Provide education, information and referrals to program participants.
- Completes follow up with clients to ensure they have received the services or advocate for them if there were denied
- Make referrals to therapeutic, legal, medical, educational, and other social services, as necessary.
- Develop and maintain relationships with private landlords.
- Assist clients with other needs connected to housing such as obtaining furnishing, home goods, and moving services through collaborating with local nonprofits.
- Keep abreast of current literature, developments, and trends in the fields of case management and community issues.
- Collaborate with local social service agencies in order to coordinate services for participants.
- Be knowledgeable and implement the Trauma Informed Care philosophy by creating a safe, therapeutic living and working environment for our clients which espouse the proactive principles of growth, change, and empowerment of the individual.
- Provide 24-hour on-call coverage on Mondays and Wednesdays to guide advocates and clients with support and basic crisis intervention.
- Maintain and report accurate and up-to-date statistics for the program.
- Assist in accessing various means of transportation for clients;
Schedule:
Monday through Friday, 12pm to 8pm.
Salary:
$21.96 to $24.00 an hour, depending on experience.
Requirements Necessary Skills:
- Desire to gain experience working with victims/survivors of domestic violence.
- Attention to detail, emotional maturity, flexibility, and ability to work independently.
- Physical ability to climb steps and lift occasional bulky items.
Requirements for Consideration:
- Bachelor's degree (B.A.) in Social Work or a related field from four-year College or university; or two years in related experience and/or training; or equivalent combination of education and experience.
- Housing/residential experience is required.
- Current driver's license and clean driving record.

Preferred Experience:
- At least 1 year of experience providing case management to victims and survivors of domestic violence.
